Veterans of the Armed Forces receive a DD 214 Form, Certificate of Release or Discharge from Active Duty, when they are separated from service.

Veterans who use Form 22-1990 to submit a claim for educational benefits should provide to the Veterans Administration a VA Certified Copy of the DD 214 Form as supporting documentation.

Veterans who have been released from active duty since October 1979 should submit the Page 4 Member Copy, which contains certain information regarding the individual’s period of service that is essential to the VA’s determination of eligibility for benefits. If the original copy is submitted, the VA will return it, but it is advised that a certified copy be provided.
